The Personal Information Protection and Electronic Documents Act (PIPEDA)
came into effect on January 1, 2004. Ball Harrison Hansell Employee Benefits
Insurance Agency Ltd. (BHH Benefits) would like to assure you that your
privacy is paramount to us.
What type of information is collected?
Personal information collected is based on the requirement of the carrier
quoting on the particular benefits package. This information is not held
at BHH Benefits, but forwarded directly to the carrier.
Company census information, held in relation to a benefits plan, does
not include names with the specific data collected.
How do we maintain files?
Files within BHH Benefits are maintained both electronically and in printed
copy.
How do we safeguard our files?
Printed files are secured within locking cabinets, stored within a locked
filing room.
Electronic files are held within our local network which is protected
by a NOVELL firewall. Files within this network are only accessible to
BHH Benefits employees through their workstation computers, which are
individually password protected.
The premises occupied by BHH Benefits are maintained by a reputable security
company.
How are old files disposed of?
Files no longer required are shredded in their prescribed time periods
in accordance with the CRA and provincial legislation.
Who is information available to?
Information gathered from our clients is utilized to underwrite policies
and assist in processing claims. These actions require that we share your
information with the carriers of these policies. In completing your application
for enrolment with a particular carrier, you are authorizing the sharing
of this information.
At no time will we sell or provide any personal information about
you to an outside company.
You have the right to ensure that any/all information we maintain on file
about you is accurate and accessible.
